If you are new to our office, we ask that you please bring the following to your visit:

• Insurance card
• List of medications (both prescription and over-the-counter) including dose, frequency, etc.
• Completed new patient forms/packet

 

Any relevant medical records, including lab results, x-rays, pap smear and mammogram results, etc., that are from another provider or results we my not have. If you are transferring from another provider outside our practice, we ask that you have your past medical records transferred to us. This will help facilitate your first visit.
